In order to attend, participants must meet the following requirements:

  • Participant can take care of all personal and self help needs

  • Participant is ambulatory

  • Participant does not harm self, others or property

  • Participant will engage in a group setting without one-on-one assistance

  • Participant has their own transportation with prompt arrival and pickup

If your child/adult meets the above criteria,  please complete the following form, noting the program you are interested in and providing a little more information about your child/adult.
